Reimbursement

SEA Personnel and State Affiliate Representatives

Reimbursement (up to $400 per person) will be available to all invited state team members (SEA personnel and state affiliate representatives). Receipts showing participant's name, amount paid, and flight schedule must be attached to and submitted with the travel reimbursement form, which is available below.

Reading First Program Directors/Designee

CCSSO is providing direct reimbursement to Reading First Program Directors/Designee only. If you fall into another category of summit attendance, you must use the SEA Personnel and State Affiliate Representatives form above.

CCSSO is providing direct reimbursement for only the following expenses:

Airfare up to $500
Ground Transportation (Taxis, parking, and/or mileage) up to $100
Hotel stay for the night on December 7 th – only if you received prior approval

In order to receive reimbursement, you must do the following.

Fill out the reimbursement form completely. There are two pages.
Attach original receipts for all expenses.

Hotel for 12/7/07 – A hotel folio showing a $0 balance.
Airfare – An itinerary with only flight information is not sufficient. There must be an indication of payment (usually the name of the payer and the last four digits of the credit card number is listed or you may submit information from a credit card statement).
Ground transportation – Original taxi and parking receipts. If you are claiming mileage, then you must indicate starting and ending points as well as the number of miles.

Please clearly indicate to whom the reimbursement is to be paid. If there are multiple entities, you must indicate such. For example, if your state agency paid for your plane ticket while you paid for your ground transportation, then we need to know that.
